We educate all across the US on insertion of ultrasound guided devices from IV’s, Midlines and PICC’s. The most important process we teach is “Which device” should I place? Device Selection for IV therapy is the first step in the insertion process. There is a lot of confusion on device selection. Some of this comes from the order we receive. We often get the “PICC” order and that is what we place. We need to look at all options for the patient. Maybe it’s a simple ultrasound guided peripheral IV or Midline. Do you assess the patient, the EHR and their vasculature and then call the Physician and say… “Hey…I really think the patient needs a ____ (IV or Midline) versus the central line”. Is the the culture you have set up in your facility? Do you look at their PMH, labs, prescribed therapy? The first thing we need to know is, what is the medication we are administering and how long will the patient need treatment. Do you ask your patient about device preference? What is the plan after discharge? Protect your patients from harm and include them in the selection process. Know your devices and know the patients therapy before you choose the right device.
